How To Apply Genuine America Visa Lottery Application Form | Guide

The only way to apply for a green card lottery / America Visa Lottery legitimately is to apply online on the State Department website. Beware of any other websites that charge fees for this free application from non-government.

The form you fill out online is called Form DS-5501, Electronic Diversity Visa Entry Form, which is only available during the open registration period of the DV.

You can't download and fill out the America Visa Lottery form. Physical entries are not approved beginning with DV-2005. The online process makes the process more secure and efficient. In addition, the State Department uses the special technology and other methods to identify individuals who commit fraud or illegal immigration or those who send multiple entries.

You should not wait until the last week of the registration period as there may be high demand and there may be delays in the website.

If you do not complete the online form, it will disqualify your entry.

Regardless of who made the entry, all your entries will be disqualified if you make more than one entry. Regardless of who made the submission, it is solely your responsibility to ensure that the entry information is accurate and complete.

No documentation (birth certificate, work experience, school records, etc.) should be sent anywhere other than electronic entry.

Instructions for Applying Visa Lottery for America 

1. FULL NAME – last / family name in letters of capital, first / given name, middle name

2. DATE OF BIRTH –  Week, Month, Year Please note that even if the date of birth commonly used in the U.S. is Month, Day, Year; you can write the date of birth in the format Date, Month, Year as typically used in most parts of the world.

3. GENDER – Male or Female

4. CITY WHERE WERE BORN – Explaining yourself.

5. COUNTRY WHERE YOU WERE BORN – The name of the country where you were born is actually in use.

6. COUNTRY OF ELIGIBILITY And CHARGEABILITY OF THE DV PROGRAM – It's the country of your birth in most situations. Many candidates, however, may be eligible to use either their spouse or one of their parents in the country of birth.


7. ENTRY PHOTOGRAPH(S) – You, your spouse and all your minor unmarried children under the age of 21, if appropriate, have recently taken digital photographs. The photograph must be a photograph of the individual. Photographs of the family or group are not recognized.

Include all your natural children, your other spouse's children, legally adopted children and stepchildren who, on the date of your application, are unmarried under the age of 21. These children must be included even if you are no longer married to the parent of the child, even if the child is not living with you and/or is not going to immigrate with you.

Do not include your children or lawful permanent residents who are already U.S. citizens.

If you are married, unless you are legally separated (i.e. there is a written agreement recognized by a court or court order), your spouse must also be included, even if he / she resides with you and/or does not immigrate with you. Of example, if you're divorced or widowed legally, you don't have a partner anymore, and you don't have to enter information about the former spouse.

In your DV request, married children and children 21 years of age or older are not eligible.
Nevertheless, according to the Child Status Protection Act (CSPA), if the DV request is made before the child turns 21 and the child turns 21 before obtaining a visa, it may be necessary for the child to immigrate with you. In such cases, he / she may be shielded from aging out and will be considered as if for the sole purpose of obtaining the visa he / she was under 21 years of age. In order to take advantage of this, however, the child must actually seek to acquire the immigrant visa within one year of the access to the visa.

Unless you fail to list all of your qualifying children and/or your partner, the entire case will be disqualified and at the time of the visa interview, the immigrant visa will be denied. Of course, if you had any kids born after you made the application and before the visa was released, you probably couldn't include those kids in the original application and they'd still get the immigrant visa.

Of course, to apply for America Visa Lottery, you don't need to be married or have children.

8. MAILING ADDRESS – In Care Of, Address Line 1, Address Line 2, City / Town, District / County / State, Postal / Zip Code, and Country.

9. COUNTRY WHERE YOU LIVE TODAY – Explaining yourself.

10. PHONE NUMBER – It's probable. However, it is recommended to put it if you have a phone number where you can be reached.

11. E-MAIL ADDRESS – It is no longer available from DV-2011 onwards. It's mandatory. Please provide the email address you have access to directly. Please do not provide the email address of someone else or a general address of the client.


At this email address you will NOT receive an official letter of choice. Nevertheless, if you pick your entry and respond to the official letter you got from the Kentucky Consular Center (KCC), you will receive e-mail follow-up correspondence from them.

12. WHAT ARE THE HIGHEST LEVEL OF EDUCATION ACHIEVED, AS TODAY? You will show which of the following is your own highest level of educational achievement: (1) primary school only, (2) high school only, (3) high school graduation, (4) vocational school, (5) college courses, (6) university degree, (7) graduate courses, (8) master degree, (9) undergraduate courses, and (10) doctorate degree.

13. MARITAL STATUS – Married, divorced, widowed, or legally separated.

14. CHILDREN NUMBER – Total number of kids. For more information, please see #7 above.

15. SPOUSE INFORMATION – Name, Birth Date, Sex, Birth City / City, Birth Country, and Photo. For more details, please see #7 above.

16. CHILDREN INFORMATION – Name, Birth Date, Sex, Birth City / City, Birth Country, and Photo. For more details, please see #7 above.

There is no need for signatures on the E-DV entry form.

Confirmation of application for Visa Lottery for America  

Once you have completed the entry successfully, you will see a confirmation screen with your name and a specific confirmation number. For your records, print this confirmation screen. To check the status online, you will need this confirmation number starting in March of next year.

If you miss the notification page (or number), if you receive an official letter from the usps, the only way to know if you were picked in the lottery. The KCC is unable to give you the confirmation page or number. The list of identified persons can not be accessed from the U.S. embassy or consulate.
